Medical Emergency Statement

I have listed the correct contact information on the registration form in case my child(ren) has a medical emergency. If there is something We Are Coaches needs to be aware of, I agree to provide the proper and necessary medical information/instructions via email or in person (or both).

In the event of sudden illness, accident, or injury which may occur while said minor is engaged in activity supervised by the representatives, agents or assignees, when neither the parents, guardian, or designated family physician can be contacted, I hereby give my consent pursuant to California Civil Code #25.8 for emergency treatment as shall be necessary under circumstances by any physician licensed under the Laws of the State of California and release and discharge We Are Coaches, Inc., and agents, or employees from any and all claims for personal injury.

Payment

PayPal accepts credit cards and e-checks. After successfully submitting your payment, you will receive an email to complete your registration. Your children are not registered until payment is complete. E-mail if you prefer to pay via check.

Cancellation Policy

 

If you need to cancel camp, you must send an email two weeks prior to camp start date. You will receive a full refund less $100 cancellation fee. If you need to reschedule your camp date, you must email no later than ONE WEEK prior to the start of camp. Any changes made between one week and 72 hours prior to the start date of camp, will receive a transferable credit for lessons or camp (good for 12 months) and will incur a $30 reprocessing fee. There will be no refunds or reschedules for any changes within 72 hours of camp. Refunds take 2 to 3 weeks to process.

Full refund will be issued for any program canceled by We Are Coaches, Inc. Activities are subject to change due to weather or water conditions. You will receive a confirmation email with 24 hours of submitting this form.

Visual Release

​

I understand that my child might be in photographs and videos that may be used by We Are Coaches and/or its photographers for marketing purposes. I grant any or all photographers and We Are Coaches the right to use and incorporate, in whole or in part, photographs or video footage taken as a result of participating in We Are Coaches programs.

Early Drop-Off

Inquire about availability 48 hours in advance.

 

Rain Policy

​

We will still meet if it is raining. If it is downpouring or thunder/lightning at 8:30 am we will postpone start time to 10:30 am unless conditions are still poor. If we cancel during mid-day, we will call you for early pick-up. If we cancel due to poor weather or water conditions, we will give each student $45 credit for a future day of camp which can be used for that season or the upcoming season.
